To start the accommodations process at BYU, you will need to fill out an intake form before making an appointment. There are two forms available, one for temporary conditions and one for conditions lasting over 6 months. You can find these forms on the Forms tab of our website. Once the intake form is complete, you can make an appointment by calling the office.

Utah Valley University

To get to the Losee Center most easily, park in Parking Lot K and go to the building to the east of the parking lot. The Accessibility Services office is on the third floor and tucked away far down the hall. This can make it difficult to find.

Accessibility Services appointments tend to fill up quickly. Try to make an appointment as soon as possible. Fill out the required forms before attending your appointment, but you do not need to fill them out before making the appointment. About Same

I started this website in 2020 after struggling to get accomodations since I was diagnosed with a life changing disease at 8 years old. Since then, I have been a fierce advocate of those with disabilities like myself. It wasn’t until I dropped out of college due to my health issues and not being able to get the help I needed, that I realized this was what I needed to do. So here I am, creating a website with all the information I wish I had when I needed it most. I wish you the best of luck and I hope this website helps you succeed in one way or another. I’m proud of you for making it this far in your education, and I don’t want anyone to stop you.
